Design of a real-time Web application for collaborative annotations in telemedicine.

In the last decades the bandwidth of private internet accesses has been increased continuously (Coffman and Odlyzko 2002, Bundesnetzargentur 2015). Video streaming and conversations, e.g. Skype, became usual for the typical internet user (Bundesnetzargentur 2016).

In different domains of medicine, e.g. in radiology, video consultation has already become widespread (Deshayes and Philippe 2000). In consultation for instance whiteboards are used to enhance the communication between patients and doctors. In order to enhance video consultation it is mandatory to make use of different collaborative consultation tools as a digital whiteboard.

Furthermore, users prefer running software within the environment of a web browser (SAAS). Downloading and installing software only is necessary if performance issues occur in the web browser or if the content has to be available offline. Hence, in the case of a medical video consultation it is not necessary to make a consultation software available offline.

A frequently used method of transmitting video streams is the WebRTC protocol. Web Real Time Communication is an API, providing protocols for real time communication between many clients using a webserver (Johnston and Burnett 2012).

Through the enhancement of telemedicine, doctors and patients save a lot of time and money. Waiting time, driving time and unnecessary visits of a doctor may be reduced. Therefore all components of the health system will be improved.

In general video consultation in the field of medicine is not common so far. The development of supporting collaboration tools as a digital White Board should be based on scientifically proved requirements in order to ensure quality, safety and performance. A digital White Board should help users to handle and collaboratively edit different streams, as notes, webcam videos, annotations to images and much more.
